SITE COMMITTEE QUESTIONNAIRE

Hi allas many of you know, the Site Committee is working on making a handbook that can be used by future Site Committee people as well as members at large. One of the things that will be included is the evaluation of the parks we visit on our rallies. If you will complete the 9 statements by typing the number matching one of 7 possible options, this will be the fifth questionnaire for 2011.

The first thing to do is hit forward on this email. Once you hit forward, this page will arrive on your screen as an email ready to be forwarded. Before forwarding, scroll down and you should see the questions. You will be able to type your responses at the end of each numbered item. To keep it simple, we're asking that you complete the statement with one of the following responses.

Remember, we only need the number:

-3= totally unsatisfactory

-2= unsatisfactory, but livable

-1= OK, but not great

0 = no opinion

1= acceptable

2= really good - among the better parks

3= outstanding - the best

The 9 question survey above is ONLY about the park. Since the parks have no control over the weather, try not to let it color your responses. For those of you who attended one of the activities at name of city, such as golfing or any other activity, we would be grateful if you could write a couple of lines about any or all those experiences as well as comments about the weather or other non park features to add to the growing knowledge bank.

We tried to make this easy by limiting the number of statements to only 9 and the number of answers to only 7 possibilities using numbers instead of words.
